Member reading statistics are not a tax invoice and they are not the amount Medium sent. Use the Partner Program payout: date, currency, and amount. Medium is the third-party payer. The Medium company on the advice is the service recipient.

Other online content on this site uses SAC 998439. Banks often apply purpose code P1101 to audio-visual and related services. Confirm that this is the code your bank wants for paid reading, rather than a production invoice under 999612.
Use 999612 only when the invoice is for video or audio-visual production. A Partner Program payout is not that invoice unless you actually billed for production.
File within 30 days after the invoice month. One quiet month does not need a blank form. File the month you invoiced or the month of the payout you are treating as the invoice, and keep those dates consistent.
